Psoriasis and eczema Treatment Protocols

User leaflet for Ornit products (Psoremiss)


Since psoriasis is a chronic condition it constantly challenges doctors striving to develop effective methods for its treatment. So far, there is no ultimate cure for the disease, but rather constant attempts to improve the symptoms and control the condition more efficiently. PSOREMISS method is a new approach to psoriasis treatment, developed in Israel.

While working on the method, the researchers took into the account the fact that psoriasis is not a mere skin disease. Therefore, the treatment is three-fold:
• local treatment of skin areas affected by psoriasis
• immune system treatment
•measures aimed at changing the patient's psychological perception of the condition

1.1 The developers of PSOREMISS method were guided by the following objectives:
* to achieve improvement of clinical state of the patient within a short time from the beginning of the treatment
* to cure the consequences of psoriasis occurrences
* to achieve long-standing remission
* to minimize adverse effects of the treatment
* to preclude even slightest depression caused by the medication
* to make the treatment pleasant for the patient
* to produce a medicine that will be effective for any severity level (light, medium or bad).
1.2 Topical treatment using PSOREMISS  method
The developers of PSOREMISS method turned to the well-known medications for psoriasis based on tar, which up to now remains the most risk-free method for psoriasis therapy. By excluding redundant fractions from tar, they developed an improved tar formula.
The resulting substance was enhanced with:
- extract of the Dead Sea gilsonite
- antifungal plant extracts
- antibacterial plant extracts
- anti-inflammatory plant extracts,
Each extract is prepared on the most suitable basis. The extracts were compiled through selection of renowned for their effectiveness botanical pharmacological products. Each of the components was tested for the nature and level of the clinical effect, which resulted in a selection of extract combinations demonstrating the highest performance.
The research continued also after the key selection had been made, which, in the long run, resulted in additional alternative formulas.
1.2.1 Psoremiss  50 and 53 (Ornit 21/23) is a combination of:
• water-based plant extracts
• natural oils
• small portion of salicylic acid
This medicine does not contain corticoids.
The product cleanses the affected skin area off the flakes and, due to the plant extracts and natural oils, softens the skin, disinfects it and produces anti-inflammatory and anti-itching effect.
The preparation is to be applied in the morning on the affected area after washing it with soap and water.
1.2.2 Psoremiss  49 (Ornit 19) is a combination of:
• plant extracts
• gilsonites
• small portion of water-based pix
This cream does not contain corticoids.

Gilsonite extract produced from the Dead Sea minerals slows down the process of cells excessive growth.
Plant extracts as a basis of the medicine have anti-inflammatory effect, while pix serves as antimitotic agent reducing the intensity of excessive keratinocytes building-up.
The preparation is to be applied on the skin in the evening, after shower and leave on for the night.
It is worth noting that formulas 19 and 21/23 can be interchangeable, i.e. formula 19 can be applied in the morning, while formula 21\23- at night, according to the patient's preference (formula 19 may leave stains to clothes and bedding).
1.2.3 Application of formulas 21 and 23 as shampoo for the treatment of regular psoriasis on the skin covered with hair (head, genitals).
The formulas exfoliate and slow down the process of cells excessive growth.
To be applied 3 times a week on wet hair.
1.2.4 Formula Р-1
To be used by medical doctor only. Not supplied through PSOREMISS.COM
1.2.5. White cream № 9
To be applied to soften the skin.

1.2.6. Treatment protocol:


1.2.6.1 Initial treatment phase (two weeks)
Ornit 19 is prescribed to be thoroughly rubbed into the affected skin areas in the morning after shower.
In the evening, after shower, Ornit 23 is to be applied in a similar manner.
The order can be changed according to the patient's preference (23 in the morning and 19 in the evening).


1.2.6.2 The expected progress
The expected result may vary from insignificant improvement of damaged skin to complete disappearance of the symptoms. Adherence to this rule provides for excellent chances to achieve sustainable remission during the major course of treatment.
If the initial treatment produced insignificant results, it may be advisable to prolong this phase for another week.
If none of the medicines available produced the desired effect, PUVA therapy may be applied along with the prescribed treatment (consult your physician!)

1.2.7 40%-cycle principle


The standard time of the topical therapy medicine contact with the skin (the time when the medicine stays on the skin) shall not exceed 40% of 24 hours, i.e. 9.6 hours.
When applied to sensitive body parts (sentinel tags, face, genitalia), the contact time shall be initially significantly less (2-3 hours) and gradually increased, day-by-day, regardless if the patient is allergic or hypo-allergic.
If it is known that the patient suffered certain allergies in the past, additional precautions should be taken, i.e. the initial contact time shall not exceed several minutes.

1.2.8 Fragmented sequence principle
If the area subject to application of the medicine  >200 sq. cm (31 sq. inch), it is recommended that different fragments shall be treated alternatively.


1.2.9 Skull skin psoriasis
In case of mild manifestation, when the "patches" are few in number and small in size, with thin flakes, the treatment may be limited to application of Ornit 21/23  3-4 times a week right after washing the hair with a regular shampoo. The product shall be applied to wet hair, massaged into the scalp, left on for 5 minutes and washed off with warm water. Conditioners and other hair-care products can be used. However, no other medical shampoo can be used in parallel.
If the flakes are thick forming incrustation, regardless their location and spread, the patient should apply formula 21(23) in the morning and cream 19 in the evening, after washing the hair with a regular shampoo. In order to remove the flakes that fell off during the washing, a regular comb should be used.
If the flakes are very thick, occlusive application of formula 19 is highly recommended. The formula is to be left on the scalp overnight (with the exception of the patients susceptible to allergies. If so, the contact time shall be increased gradually, or limited to several hours).
Very often, when psoriasis affects the scalp, it spreads over behind the ears. Similar treatment is prescribed taking account of anatomic features of these skin areas (the time of the skin exposure to the cream may need to be limited).
1.2.10 Facial psoriasis
Psoremiss  creams can be used for treatment of affected areas on the face, including eyelids, external and internal corners of the eye, except for semimucosa of the lips and nostrils.
Some components have photosensitizing effect, hence after applying the formula in the morning with subsequent exposure to the sun, itching and erythema symptoms may get worse. This can be prevented by using sunscreens, factor 60-100.
It is recommended to abstain from visiting suntan salons.

1.2.11 Psoriatic blisters on the ears and outer hearing organs are generally most painful.
The affected areas are treated as described above with formulas 19, 21 and 23, applied very thinly to prevent the cream from getting into the auditory tube.


1.2.12 Psoriatic "patches" and plates on the arms, legs, front and back parts of the chest, abdomen and buttocks are normally treated with Ornit creams 19 and 23.
It should be mentioned that in case of treatment for thick plates frequently recurring on the elbows, knees and lower back, occlusive application of formula 19 is recommended to be left overnight.
When taking a shower, it is important to soak the affected skin in these areas with warm water and rub it with rough sponge, naturally, avoiding extreme rubbing of the blisters to prevent occurrence of Kebner effect.


1.2.13 When treating psoriatic symptoms on female breasts, the creams should not be applied on mammary areola (the treatment is not recommended for breast-feeding mothers).


1.2.14 In the armpits, femoroingiunal parts, between the buttocks and under the breasts (inverse psoriasis), where the skin is thinner and more sensitive, the cream should initially be left on for not more than 2-3- hours. Subsequently, the exposure time may be prolonged depending on the patient's tolerance.
Psoriatic symptoms may be preceded by mycosis, such cases requiring preliminary topical ± general anti-mycosis treatment. (Consult your physician!)


1.2.15 Psoriasis on the hands (back of the hands) requires similar treatment. However, in this case formula 23 should be applied repeatedly after each time the patient washes his hands during the day.
Also, since the hands are exposed to the sun, photosensitivity of the skin needs to be taken into consideration and, if necessary, protect the hands with high-factor sunscreen cream when leaving home in the morning (this problem may occur in relation to any skin area exposed to the light).


1.2.16 Ornits series should not be applied to semimucosa of the anus, mouth and female genitalia..
When treating sores on the skin of the penis, or the scrotum, the creams should be washed off after 2-3- hours.


1.2.17 Nail psoriasis is a complicated therapeutic problem.
It is a known fact that medicines in the form of creams, lotion or gel penetrate under the nail only insignificantly, or do not penetrate at all.
This is why it is recommended to apply formula 19 and 23 in the morning and in the evening, accordingly, to periungual skin (periungual folds).
Regardless the peculiarities of the affected skin area, there are certain rules that have to be observed when using the products:


* The preparation should be applied very thinly, and rubbed it gently so that the medicine is completely absorbed within 5 minutes.
* The preparations should be applied only on the affected area so that healthy skin is avoided.
1.3 Topical external application is complemented with pectoral system medication, using the substances helping to balance the keratinization process and to regulate the immune processes in skin tissues (vitamins A, D, F), and antioxidants affecting neuropsychic system (vitamins B, neuroleptics, sedatives).
Also, the patients are recommended to keep diet that excludes sympathomimetic products (alcohol, caffeine, spice), and is worked out individually taking account of any other conditions the patient suffers (cardio-vascular diseases, digestive disorders).
